Waiver: 
I, parent, give permission and acknowledge that this event is a Revolution Academy PTO hosted event and no refunds can be given after registration ends.  If the student is unable to attend they will still receive the t-shirt.  Registrations are accepted until Thursday October 1st, no exceptions.
Football Player: My daughter has permission to sign-up and play Powderpuff Football for Revolution Academy.
Cheerleader: My son has permission to perform as a Cheerleader for the Powderpuff Football game for Revolution Academy.
